Southwest Florida COVID-19 vaccination sites and information

Beginning April 5, all individuals 18 and older will be eligible to receive any COVID-19 vaccine.

Sixteen and 17-year-olds will also become eligible April, but can only receive the Pfizer vaccine according to FDA guidelines.

The state is continuing to follow all FDA issued Emergency Use Authorizations guidance when administering the COVID-19 vaccine. At this time, the Pfizer vaccine is the only vaccine currently authorized for individuals under 18.

Anyone under the age of 18 receiving a vaccine must be accompanied by a guardian and complete the COVID-19 vaccine screening and consent form. The form is available here.

All state-supported vaccination sites that offer the Pfizer vaccine will administer the vaccine to individuals under the age of 18 who are accompanied by a guardian and have the completed COVID-19 vaccine screening and consent form.

Friday, the Florida Division of Emergency Management announced that the four federally-supported vaccination sites in Jacksonville, Orlando, Tampa and Miami will transition to the one dose Johnson and Johnson vaccine on Tuesday, April 6.

The state has been able to make this change as it receives an increased allocation of the Johnson and Johnson vaccine this week. After Monday, April 5, the first dose of the Pfizer vaccine will no longer be offered at the four federally-supported sites.

The four federally-supported sites will administer up to 3,000 one dose Johnson and Johnson per day, per site.

In order to reduce confusion regarding first and second doses due to ongoing changes at the federally-supported vaccination sites, the four federally-supported sites will begin only offering the Johnson and Johnson vaccine. This will streamline the vaccination process at the sites and eliminate the need for individuals to return for a second dose.

All individuals who received their first dose of the Pfizer vaccine at the federally-supported sites are guaranteed to receive their second dose at the federally-supported sites. The four main sites and the satellite sites will continue administering second doses of the Pfizer vaccine for individuals who received their first dose at these sites.

Once the satellite sites finish administering all second doses, they will no longer be operational.

Other state-supported vaccination sites continue to offer the Pfizer vaccine. The Division continues to encourage individuals to visit state-supported vaccination site, in addition to the federally-supported vaccination sites.

Each of the four federally-supported vaccine sites in Jacksonville, Orlando, Tampa, and Miami are able to administer 3,000 first doses of the Pfizer vaccine per day. To make this change, Governor DeSantis directed the Florida Division of Emergency Management and Florida Department of Health to allocate state vaccine allocations to these federal sites.

Additionally, the four federally-supported sites will remain open for four additional weeks. These sites were previously expected to close on April 28 and will now close on May 26. To keep these sites open, the sites will utilize state vaccine allocations.

These large, federally-supported sites are a critical part of the state’s efforts to expand vaccine access as more Floridians become eligible for the vaccine. Each of these sites was opened in order to provide the vaccine in underserved communities, and Florida was the first state in the nation to open large federal sites using a walk-up model and was the first state to use a hub and spoke model to reach more underserved areas. To date, these sites have vaccinated more than 300,000 individuals.

Originally, the federally-supported vaccination sites were designed to remain in their communities for eight weeks, and in order to ensure all individuals who visited the site were able to receive second doses prior to the sites ceasing operations, the federally-supported sites were planning to only offer second dose shots March 24 – April 14. Since March 24, the state has been re-allocating unused federal vaccine to the sites to provide up to 500 additional first shots per day.

Floridians are encouraged to also visit state-supported vaccination sites to receive a vaccine. State-supported sites can vaccinate all eligible individuals under Governor DeSantis’ current Executive Order, including those deemed medically vulnerable. Walmart / Sam's Club Vaccination Information
Walmart and Sam’s Club announced a new COVID-19 vaccine milestone: Immunizations are now available to customers and associates in all of their pharmacy locations nationwide, including the 383 Walmart and Sam’s Club pharmacies in Florida.

Vaccines are being administered via both walk-up and scheduled appointments, providing convenient options for customers to get protected against COVID-19 where they already shop for groceries and health care essentials.

Winn-Dixie Vaccination Information
Southeastern Grocers, Inc. (SEG), parent company and home of BI-LO, Fresco y Más, Harveys Supermarket, and Winn-Dixie grocery stores, partnership with the Florida Department of Health to distribute approximately 8,100 free Moderna COVID-19 vaccines throughout the state of Florida.

As a preferred retail partner in the fight against COVID-19, SEG will begin to administer the free Moderna COVID-19 vaccine on Thursday, Feb. 11, to eligible recipients in select Winn-Dixie, Fresco y Más, and Harveys Supermarket locations through the Federal Retail Pharmacy Program.

The grocer will initially offer the free vaccines to health care workers, residents and staff of long-term care facilities, and adults ages 65 and older in accordance with Florida health authority guidelines, while supplies last.

Qualified individuals are encouraged to visit these sites: Frescoymas, Harveys Supermarkets, and Winn-Dixie.

Publix Vaccination Information
Publix is suspending the administration of the Johnson and Johnson COVID-19 vaccine in accordance with guidance from the CDC and FDA. According to the CDC and FDA, as of April 12, more than 6.8 million doses of the Johnson & Johnson (Janssen ) vaccine have been administered in the U.S. CDC and FDA are reviewing data involving six reported U.S. cases of a rare and severe type of blood clot in individuals after receiving the Johnson and Johnson vaccine.
While a rare side effect, individuals who have received this vaccine should contact their health care provider if they develop a severe headache, abdominal pain, leg pain or shortness of breath within three weeks after vaccination.
CDC will convene a meeting of the Advisory Committee on Immunization Practices (ACIP) on Wednesday to further review these cases and assess their potential significance. FDA will review that analysis as it also investigates these cases. Until that process is complete, both are recommending a pause in the use of this vaccine out of an abundance of caution.

Publix will open its online reservation system every Friday at 7 a.m. for Moderna COVID-19 vaccinations appointments in Florida for the following week beginning with the scheduling opportunity on Friday, April 9.

The online reservation system will no longer open on Mondays to schedule vaccination appointments in Florida.

Although individuals ages 16 and older are eligible in Florida, the Moderna vaccine is only authorized for those 18 years of age and older.

Appointments
Vaccinations are provided to individuals ages 18 and older by appointment only, while supplies last, through the online reservation system at Publix COVID-19 vaccine.

Appointments cannot be made by calling Publix or the Publix Pharmacy.

Individuals can use the online reservation system to learn if appointments are available at their nearby store.

Moving forward, the online reservation system will open at 7 a.m. Eastern time on Wednesdays and Fridays for Florida appointments as follows, as long as Publix continues to receive vaccine doses and barring unforeseen circumstances.

Appointments may not be available in every county during every scheduling opportunity:
Customers with appointments for their second dose of the Moderna vaccine are not impacted by any changes in scheduling opportunities and should arrive at the COVID-19 vaccine check-in area at their scheduled date and time.

Locations
Publix administers the vaccines in all 43 Florida counties with Publix Pharmacy locations. The company continues to communicate with the states in its operating area to provide COVID-19 vaccinations as part of their distribution efforts.

Customers with health insurance will need to bring their insurance card to their scheduled appointment. Medicare members should bring their red, white and blue Medicare Part B card. Customers without health insurance will need to provide their driver’s license or Social Security number.

CVS
CVS Health announced that COVID-19 vaccine appointments for adolescents ages 12 to 15 are now available for scheduling at more than 5,600 Pharmacy locations nationwide, following Food and Drug Administration (FDA) Emergency Use Authorization of the Pfizer-BioNTech vaccine for this age group. Starting May 13, participating CVS Pharmacy locations will begin administering the Pfizer-BioNTech vaccine to this newly eligible population. Parental or legal guardian consent is required, and children must be accompanied by an adult.

Because the Pfizer-BioNTech vaccine is the only FDA-authorized COVID-19 vaccine for adolescents in this age group, patients are encouraged to schedule an appointment online at cvs.com or through the CVS Pharmacy app to ensure availability. Walk-ins are also accepted. The scheduling tool will only display appointments at CVS Pharmacy locations that have the Pfizer-BioNTech vaccine once the patient’s age is provided.

CVS Health accepts walk-in COVID-19 appointments at CVS Pharmacy locations across the country, including 782 locations in Florida, with no appointment necessary. Same-day scheduling, including appointments as soon as one hour from time of scheduling, is also available at CVS.com. The company’s COVID-19 vaccine scheduler is updated throughout the day to account for same-day cancellations.

LEE COUNTY
The Florida Department of Health in Lee County (FDOH-Lee) COVID-19 vaccination site relocated from the old Sears at the Edison Mall to FDOH-Lee offices.
FDOH-Lee is offering COVID-19 vaccinations to Florida residents and persons providing goods and services to Floridians as prescribed by the Food and Drug Administration. Currently, this includes individuals ages 12 and up.

Lee Health
Lee Health is permanently continuing walk-in vaccine availability for the community. Anyone can get a vaccine at no cost by visiting the COVID-19 vaccination clinic at Gulf Coast Medical Center. The clinic is open Tuesday-Friday from 7 a.m. to 3:30 p.m. Appointments can also be made by visiting www.leehealth.org.
In an effort to further increase vaccine availability to the community, Lee Health’s vaccine clinic will also be open on Saturday, May 29, from 9 a.m.-1 p.m.
Lee Health is now offering the opportunity for all patients 18 and older to choose which COVID-19 vaccine they receive, based on current availability. Lee Health currently has Pfizer, Moderna and Johnson & Johnson vaccines immediately available for anyone wishing to get vaccinated.
Those aged 12-17 are still only able to receive the Pfizer vaccine and must have a parent with them when they receive their vaccination.
To receive your COVID-19 vaccine at the clinic, please go to the main entrance of Gulf Coast Medical Center and check-in at the front desk. Please bring personal identification and your insurance card if you have one. There is no cost for the vaccine.
A mask is required to enter any Lee Health facility.

Millenium Physician Group has received 1,000 COVID-19 vaccines. Millenium's vaccination plan follows Florida Department of Health guidelines regarding those who are 65 and older. It is important to first protect the most vulnerable patients in this age group with multiple underlying conditions, which place them at most risk for COVID-19 complications.

Millenium has already begun calling those who qualify. Please understand there is no need to call your doctor's office. This will not change your ability to receive the vaccine as Millenium is continually evaluating all patients' health and will continue to keep you updated on future allocations. In the meanwhile, you are encouraged to get vaccinated at any of the locations made available by the state.

Hope Lutheran Church in conjunction with the Lee County Health Department will be offering COVID vaccines to those who are eligible in the state of Florida.

The vaccine clinic will offer Pfizer vaccine that requires 2 shots on Thursdays April 15 and May 6 from 9 am to 2 pm, with the last appointment slot being 1:30 pm.

CHARLOTTE COUNTY

The Florida Department of Health in Charlotte County (DOH-Charlotte) will be offering vaccines to all eligible Florida residents. The clinic will be held at the Department of Health, 1100 Loveland Blvd., Port Charlotte, starting Monday, May 24, and will run Monday-Friday.

Appointments are recommended but not required. Walk-ins will be accepted, while supplies last. To make an appointment, call 941-624-7200.

The Moderna COVID-19 vaccine will be administered 9-:00 a.m. – 4:00 p.m.

Health Hero’s will also be on site to provide the Pfizer vaccine for school age children 12-17, from 2:00 p.m. – 5:30 pm..

COLLIER COUNTY
The Florida Department of Health in Collier County (DOH-Collier), will provide the Moderna vaccine Monday - Friday from 9am-4pm at DOH-Collier Naples site building H, 3339 East Tamiami Trail, Naples. No appointment needed. The Moderna vaccine is authorized for persons age 18 years and up.

• Walk-up. No appointment needed.
• Must be 18 years and older.
• Face masks must be worn while waiting.
• No referral needed. No cost.

Clients will be asked to complete a registration form prior to receiving the vaccine. DOH-Collier is offering the opportunity for homebound clients to receive the vaccine. Individuals must call the DOH-Collier call center at (239) 252-6220 to determine if they qualify for the homebound program.

In accordance with the State of Florida Department of Health Public Health Advisory, a COVID-19 vaccine should be provided to any eligible individual who is a Florida resident or who is present in Florida for the purpose of providing goods or services for the benefit of residents and visitors of the State of Florida.

How to demonstrate Florida residency:
• Valid Florida Driver License or Florida Identification Card OR
• Two of the following items: o A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement. (Copies of deeds can be found on the Collier County Property Appraiser website https://www.collierappraiser.com/

North Collier Fire Rescue is offering the two-dose Moderna vaccine to residents 18 and older on April 22 and May 20.
When residents register for their first dose via the link below, a second dose appointment will automatically be made.
1st Dose: Thursday, April 22, from 3 to 7 p.m.
2nd Dose: Thursday, May 20, from 3 to 7 p.m.
Appointment times are the same for both the first and second dose on each date.
Vaccinations will take place at Naples Church, 10910 Immokalee Road, in Naples. The church is located on the south side of Immokalee Road, 2.5 miles east of Collier Blvd./CR 951.

NCH
The NCH COVID Vaccine Clinic is now accepting appointments for the Pfizer COVID-19 vaccine for all community members ages 12 years and older at a new vaccine clinic site in Naples.
The Food and Drug Administration has expanded the emergency use authorization (EUA) for the Pfizer COVID-19 Vaccine to include adolescents 12 through 15 years of age. Individuals 12-18 years of age must be accompanied by a guardian at the time of vaccination.
Vaccines are offered at a new location, the NCH Eagleview Building, 2450 Goodlette Rd, Suite 201, in Naples.

Marco Island
The City of Marco Island will be concluding its vaccination clinics by the end of May.
The month of May will be dedicated to providing second doses of the Moderna vaccine. If you are a Marco Island resident over the age of 18 seeking a vaccine and would like a vaccine this month, please register at http://vaccine.cityofmarcoisland.com.

The City has now completed its vaccine clinics for the first doses of the COVID-19 vaccine. The city says appointments for the second dose will continue to be filled until May 14th. The city's database will no longer accept new registrations. On their website, the city says the second dose vaccinations will take place at Mackle Park every Friday starting April 23rd until May 14th. The second doses are meant for those who got their first dose at Mackle Park.

The City of Marco Island is provided vaccines by the Department of Health which is given the Moderna vaccine. The Johnson and Johnson vaccine will not be available. If you are waiting for the Johnson and Johnson (Janssen) vaccine, you should not register through the City’s vaccine database.

City of Naples will open vaccine reservations at 10:00 am EST until the first 500 calls.

To make a reservation, please call: (239) 260-3087.

Vaccinations will be given from 9:00 am to 1:00 pm at Fire Station One on 835 8th Ave S., Naples, FL 34102.

The City will provide a phone call to message service that will accommodate 100 concurrent callers at a time. The 101 callers will receive a busy signal and should try to call back. Calls must be made within the period that the reservation system is active.

Due to the limited number of vaccines the City will receive, only the first 500 callers will be placed in a “registered” group who will then be contacted by a City employee to verify eligibility and set up an appointment. Please be patient.

Once the caller is in the messaging system, the callers will be asked only to leave their name as it appears on their government-issued identification and a call back number! No additional message or information should be left. This will reduce the time callers are in the queue, allowing more callers to connect before receiving the busy signal.

They ask you to leave only your name and call back number.

A City of Naples staff member will call each person back in the order that their call was received. Please be aware that City staff will never ask for personal information (other than the date of birth for eligibility and the consent form), financial insurance information, or social security numbers on the phone. Any information required to administer the vaccine will occur at the vaccination site.

Callers who are not able to get through will have another opportunity to register. Please continue to check the City’s website for future times and dates when the reservation system will be active.

Naples Police will confirm your appointment, and registration paperwork will be provided to you. If you arrive too early for your appointment, you will not be allowed to enter the checkpoint site.

Once your appointment time arrives, please bring the completed paperwork into the Fire Station apparatus bay to the registration table. Face Masks are required. If you do not have a face mask, one will be provided. Your paperwork will be checked, and you will receive your vaccination card.

Once all paperwork is confirmed, and you have signed the consent form, you will move to the next available vaccination table to receive the vaccine.

After the vaccine is administered, you will be required to go to the observation area for 15 minutes. Please advise onsite Firefighters of any issues you may be experiencing from the vaccine.

After you have been in the observation area for 15 minutes and have shown no signs of a reaction, you will be allowed to leave the site. Please follow the directions given by Naples Police.

Remember you are receiving the Moderna vaccine and will need to return to this vaccination site 28 days later for your 2nd vaccination.


DESOTO
Beginning April 27, COVID-19 vaccine appointments will no longer be required at the County’s vaccination site at the Turner Agri-Civic Center in Arcadia.
The number of vaccine appointments has decreased significantly since Governor DeSantis lowered the age requirement earlier this month. For this reason, pre-registration is no longer necessary.
For those interested in receiving a vaccine, health officials will be administering 1st-dose vaccines, on a first-come, first-served basis, starting next Tuesday, between 11 a.m. and 2 p.m.

COVID-19 VACCINE AVAILABILITY

No appointment necessary. If an appointment is preferred, residents can still register using the state’s vaccine registration portal by visiting, https://myvaccine.fl.gov or by calling 866-201-7013. Appointments will be scheduled between 10 and 11 a.m.

The Florida Department of Health and the Florida Division of Emergency Management have partnered to offer mobile vaccination units at several locations in Hendry County. The community will have the opportunity to choose from the Pfizer Vaccine for ages 12 and older or the Johnson & Johnson Vaccine for ages 18 and older.

Please note, this is a great opportunity for parents to have their children ages 12 to 17 years vaccinated with the Pfizer Vaccine! (Parent must be with the child and sign a consent form).
