Free program for people with chronic illness
WFTX - Chronic Disease Self Management program is sponsored by the Southwest Florida Community Foundation.
It's a free program for people with high blood pressure, emphysema, asthma, heart disease and diabetes.
This program teaches the person with a chronic illness how to set up an action plan for themselves, how to solves problems related to their chronic illness and how to make the best decisions for their health.
It also teaches you how to deal with chronic pain, fatigue and stress associated with having a chronic illness.
The program also emphasizes the importance of eating healthy foods and the importance of exercising regularly.
All participants are provided with a free book entitled “Living a healthy life with chronic conditions” by Kate Lorig.
